This notice will help you understand how we collect, use, share, retain and safeguard your information  or ‘personal data’.

It sets out your rights ie on what data we hold about you, how it is processed and how you can place restrictions on the use of your data

Personal Data

When you attend for an initial consultation you give us information on your name, address, date of birth, contact details, health problems and medical history. On each subsequent visit we record details on how you are feeling, how you responded to the last treatment and how we treated you this time.

We keep this information in paper format in locked cabinets at Teignmouth Osteopathic  Practice. We only ask you for an email address if you would like to receive our Practice Newsletters which come out once or twice a year. Your email address is stored on our computer and you are free to ask for it to be removed at any time.

Printed receipts are also stored on our computer.

We do not share your information with anyone outside of the practice. The only person that sees your information is the osteopath/s that treat you and the data controller, who is Sonia Mistely.

Why we need your personal data

We will use all the information you give us to help formulate a diagnosis and then create an appropriate treatment plan for you. We can see how you are progressing with treatment and continue or alter our approach as necessary to deliver best possible care.

If we need to contact your doctor or other health professional with regards to your case we will always seek your consent and clearly explain why and what you are consenting to. 

We will only use your data for the purpose for which it was collected.

Data retention

We retain all patient records for 10 years after their last consultation/treatment with us. In the case of children we retain their records until they are at least 25 years old. After these periods patient records are destroyed by incineration.

If you return to the practice after a 10 year gap we will class you as a new patient and take a case history again so we are fully up to date.

Your Rights

You have legal rights governing the use of your personal data under the Data Protection Act 2018. These are: to understand what data is held about you, for what purpose, how it is collected and used, with whom it is shared, where it is located, to object to it’s processing, to have data corrected if inaccurate, to take copies of data and to place restrictions on its processing. You can also request to have your data deleted but by law we have to keep your records for a minimum of 8 years, or in the case of a child, until they are 25 years old.

If you feel we are mishandling your personal data you have the right to complain. This should be sent to the ‘data controller’ who is:
